Phase II Conditioning Program

The Phase II Conditioning Program is medically supervised rehabilitation that begins within the first year after experiencing a cardiac event. It is designed to help those who have had coronary bypass surgery, a heart attack, or stable chest pain. It is available to those who have been referred by their physicians after completing a recent stress test.

The Phase II Program includes:
- 36 sessions offered over a 12-week period
- Medically supervised exercise tailored to a patient's physical capacity and according to physician's specifications
- Instruction on how to exercise properly, safely, and effectively
- Exercise and activities based on each patient's progress
- Regular progress updates provided to the patient and his or her referring physician
- Storage of each patient's progress records
- Any conditions requiring attention are promptly directed to your doctor.

Some or all of the costs for cardiac rehabilitation may be covered by Medicare and most insurance plans. However, you should check with your personal policy to learn if you qualify.
